In order to determine your eligibility for free or low-cost services, you will need to bring the following:
Child Health and Disability Program (CHDP)
Every Woman Counts (formerly the Cance Detection Program, or CDP)
Private Health Insurance
Early Access to Primary Care (EAPC)
Annual Physical Exams (for all age groups)
Vaccinations, and much more
We are fully equipped to treat all minor medical and surgical emergencies on a walk-in basis. These emergencies include:
1. CA Driver's License
2. Social Security Card
3. Two most recent pay stubs
4. Recent tax returns
5. Birth Certificate
6. Utility bill for proof of residence
7. Other documentation if applicable (Certificate of Naturalization and Bank Statements, vehicle registration for all vehicles.)
At Foothill Community Health Center could start with a phone call. We provide high-quality medical, dental, and counseling services. We also provide specialty services, including obstetric-gynecologic (women's health), podiatric (foot), and optometry (eye care) services.
When you call, our registration team members will assist you in identifying your eligibility for free or low cost medical services and or/insurance. The registrar will make an appointment for you to meet with the appropriate health care provider depending on your individual healthcare needs. We know that illness and injuries cannot be planned in advance. Please walk-in to one of our health clinics and we will take care of your health concerns as quickly as possible. You may also call us at (408) 729 - 9700 to schedule an appointment. Our staff speak Spanish and Vietnamese. In addition, we provide translation services for over 100 different languages.

There will be forms or an application form to fill out in order to assist you through the enrollment process of the different insurance available. All forms/ application will need to be completed before seeing our medical/dental provider.
