We accept many private insurance plans. If you are uninsured, we offer care based on a sliding fee scale. Eligibility for these programs can be assessed at our clinics. Call us to see how we can help you.

A pay stub or proof of income (if unemployed, bring with you a stub showing proof of the benefits you are currently revceiving, for example: unemployment insurance, VA benefits, or, if none of these apply, a signed letter indicating the nature of support you are receiving from a family or social connection.

We Welcome Walk-ins!
We know that illness and injuries cannot be planned in advance. Please walk-in to one of our health clinics and we will take care of your health concerns as quickly as possible. You may also call us at (408) 729-9700 to schedule an appointment. Our staff speak Spanish and Vietnamese. In addition, we provide translation services for over 100 different languages.
Becoming a patient at San Jose Foothills Health Center could start with a phone call. We provide high-quality medical, dental, and counseling services. We also provide specialty services, including obstetric-gynecologic (women's health), podiatric (foot), and optometry (eye care) services.
When you call, our registration team members will assist you in identifying your eligibility for free or low cost medical services and or/insurance. The registrar will make an appointment for you to meeet with the appropriate health care provider depending on your individual healthcare needs.
